Here’s all you need to do to nominate YOUR favorite teacher:

• Fill out the nomination form attached and fax to 903-463-9816.  This form may also be completed by fellow teachers, parents and students.  The more information received about the nominee, the better opportunity the nominee has to be chosen the KATY Country and MADROCK Teacher of the Month.

• Winners will be chosen once per month, September 2012 through May 2013.

• Each month, judges will choose the winning teacher and that person will receive a plaque and check for $500.

• Each month’s winning teacher is invited to the KATY County and MADROCK  studios for a live interview and is presented the check and plaque.

• All Teachers of the Month will be honored at the end of the school year when the Teacher of the Year is announced at a special banquet.  The Teacher of the Year will receive another check for $500 and plaque.
